Congratulations on your acceptance to Columbia University in Paris at Reid Hall for summer 2008!
You have a lot to do before you leave for Paris. Below you will find links to forms and information. Please read ALL information carefully. It is extremely important that you submit all forms by the stated deadlines; not doing so could jeopardize your participation in the program.
The items listed below are of primary importance as you begin your preparations. Please read the Reid Hall Summer 2008 Handbook carefully, as it will answer many of your initial questions. You must not hesitate with beginning your visa process. Please read all of the instructions carefully.
The items below are due at our office by April 21, 2008
unless otherwise noted. This means that we must receive them on or before April
21, 2008. Most of the items below need to be received as a hardcopy
by mail or scanned and emailed,
since we need an original signature or document for our records.
However, there are a few items which
are informational and do not require a signature,
so you have the additional option of sending them as a regular email. The
parentheses next to each item indicates the way it needs to be received in our
office. All items may be mailed
or physically delivered to our office in hardcopy form.
- Intent to Enroll and Program Fee Form with $300 check (hardcopy via mail)
- 4 passport photos (hardcopy via mail)
- Course Pre-Registration Form (mail or scan)
- Participant Information Sheet (mail or scan)
- Terms of Participation (mail or scan)
- Copy of name and signature pages of passport showing face clearly (mail or scan)
- Housing Questionnaire (mail or scan)
- Transcript Request Form (Visiting Students only - mail or scan)
Due July, 2008:
- Tuition Payment, if applicable
